Participants agree to:
Treat Care Encompass staff with courtesy and respect.
- Inform Care Encompass about their expectations and how they wish the support to be delivered to meet their needs.
- Communicate with Care Encompass if they have any concerns about the support being provided.
- Provide Care Encompass with a minimum of 24 hours notice if they cannot attend a scheduled appointment, including attending a Lifestyle Centre. If 24 hours’ notice is not provided, Care Encompass’s Cancellation Policy will apply.
- Give Care Encompass 30 calendar days’ notice in writing if they wish to end the Service Agreement, and immediately advise Care Encompass if the participant’s NDIS plan is suspended or replaced by a new NDIS plan or if the participant stops being a participant in the NDIS.
Rights and Responsibilities of Care Encompass
Care Encompass agrees to:
- Treat the participant with courtesy and respect.
- Provide support as listed in the Service Summary (Quote) which meets the participant’s needs at the participant’s preferred times where possible.
- Protect the participant’s privacy and confidential information.
- Provide full information regarding additional costs (activity fees) related to items not covered by the participant’s NDIS package and not included in the Service Summary (Quote), but necessary to meet the participant’s goals. These costs include entrance fees, event tickets, meals, etc.
- Communicate openly and honestly with the participant, resolve problems quickly, and respond to feedback and complaints in a timely manner.
- Consult the participant on decisions about how supports are provided.
- Provide the participant a minimum of 24 hours notice if Care Encompass needs to change a scheduled appointment to provide support.
- Provide support in a manner consistent with all relevant laws, including the NDIS Act 2013 and rules, and Australian Consumer Law.
- Keep accurate records of the support provided to the participant.
- Give the participant 28 days’ notice in writing if Care Encompass wishes to end the Service Agreement.
